Ilanka Community Health Center Care Coordinator

The Native Village of Eyak is accepting applications for an ICHC Care Coordinator.

The ICHC Care Coordinator is a regular, part-time (36hrs/week)position with full benefits package.

The Ilanka Community Health Center Care Coordinator works collaboratively with the Case Manager, physicians, staff and other health care professionals to ensure that appropriate care is provided to all patients.  The Care Coordinator ensures the patient is guided through their referral and outreach experience in a proactive and helpful manner.   Ilanka Community Health Center promotes a team-based approach to care.

Experience:
Minimum two-year office experience, preferably in a clinical/patient setting. Experience with Electronic Medical Records preferred. 

Education Requirement:
High school diploma or equivalent
